2009-2010 Player Registration form

Please register online for the 2009 - 2010 season! Filling out this form online will allow us to better prepare for tryouts and make the experience more enjoyable for all involved. Please feel free to contact us with any questions or problems.

In addition to our registration form, NERVA (New England Regional Volleyball Association) now requires that you register on their website before tryouts as well. Please print out and bring our Medical Release Form to tryouts with your $50 check made payable to Central Mass Juniors Volleyball Club. The NERVA fee will be paid by the club upon making a team.

Release and Consent form

My daughter has my permission to participate in training, competition, tournaments and travel sponsored by The Central Mass Juniors Volleyball Club. I approve of the coaches and Directors of The Central Mass Juniors Volleyball Club realizing that they are serving to the best of their ability and in consideration of the benefits to be derived by the participant concerned, I hereby voluntarily waive any claim against The Central Mass Juniors
Volleyball Club, it’s Officers, Directors and/or agents for any and all causes which may arise in connection with this activity. I certify that the participant has full medical insurance with the company listed above. I also certify that the participant has had a complete medical exam in the last calendar year and is medically cleared to fully participate with no restrictions.

You understand and intend that your clicking on the "Submit Registration & Consent" button shall operate as your digital signature as parent and/ or legal guardian of the above captioned player.

What do I need to do?

Bring a signed CMass Medical waiver to tryouts
Show up to the gym 30 minutes prior to tryout to handle registration, help set nets up, warm up, etc.
Bring check for $50 made payable to Central Mass Juniors Volleyball Club.

What am I paying for?

Fees paid to CMass cover the following items:

Gym fees
Tournament cost
Nominal coaches fee
Uniforms
NERVA membership fee
Volleyballs, net systems, medical kits, etc.

Gym space has become harder and harder to find. If you know someone who can procure additional gym space for CMass, please let us know. We can use space for additional tournaments, practices and clinics for the players who do not make our teams.
